Please note: New enrolments for our Spring Vacation Care period will close on Thursday 24th September. Any enrolments received after this date, with an intended start date during the Vacation Care period, will incur a $5 processing fee per child.

Bookings close on Sunday 20th September. Any booking requests received after this date will attract a $5 late booking fee per child.
Completing this form does not guarantee a booking - bookings are only confirmed once they appear in your Xplor Home App.
Please allow 2 business days for your booking request to be processed and check your Xplor Home App to verify that your requested bookings have been approved.
Active OOSH will not process any bookings for families who:
- Are not enrolled at the service
- Have not set up Direct Debit
- Have more than $100 owing on their account (unless this is their weekly fee)
- Have not provided Active OOSH with all required documentation (e.g. medical plans)

Programmed Experiences

The excursions, incursions and service activities listed below are those for which you are giving your child / children permission to participate in by signing this form. The fee for all Vacation Care days is $90 (before your individual Child Care Subsidy entitlement is applied).

Active OOSH conducts thorough risk assessments for each excursion and incursion, including travel. Should you wish to view any of the risk assessments for the activities listed below, please contact your service.

To facilitate child-led play and build on child interests, the children may participate in service activities that are not included in the programmed activities listed above. This may include cooking activities (where children will be able to eat what they cook); beauty spa activities (where children may have their nails painted, faces painted and hair styled); water play; science experiments; and sensory / messy play.

SERVICE DETAILS
During the Vacation Care period, Active OOSH Belair estimates that between 70 and 123 children will be booked into the service each day. If 123 children are booked in, all additional booking requests will be placed on the service wait list. The service follows a ratio of at least 1 educator to 15 children at all times. All incursion and excursion venues are clearly identified under 'Programmed Experiences' should parents or carers need to collect their child / children directly from a venue. Active OOSH ensures that all buses take the quickest, shortest and safest route to all venues.

I agree to provide my child / children with recess, lunch and a refillable drink bottle each day.

I understand that my child / children must wear sun safe clothing and fully enclosed shoes each day.

I understand that if my child attends the service without a hat, an Active OOSH Educator will supply my child with an Active OOSH hat and $10 will be charged to my account.

I will ensure all items of clothing and belongings are clearly labelled with my child's name.

I understand that service activities and incursion activities are held onsite at Active OOSH Belair 34 Carolyn St, Adamstown Heights, NSW 2289.

I give permission for my child / children to be transported by Active OOSH vehicles or hired vehicles to all excursion venues. I understand that all vehicles are fitted in accordance with legal requirements for seatbelts or safety restraints and that more information about the requirements for each vehicle is available to me in the excursion risk assessment.

I am aware that Active OOSH has a 24-hour Cancellation Policy. 

I am aware that the service closes at 6pm and understand that I will be charged a late fee of $10 per child for every 5 minutes or part thereof past 6pm until I collect my child / children.

I understand that if I claim CCS, I will not be eligble to claim my Child Care Subsidy (and will be required to pay full fee) if:

My child is a new enrolment and is absent on their first booked day, or

My child has not attended in the last 14 weeks and is absent on their last day

l agree that my child / children will have sunscreen applied prior to arriving at the service and understand that the service will follow the consent given within my child's enrolment form for my child/children to have sunscreen applied while at the service.

I understand that Active OOSH educators with the appropriate training and qualifications will provide medical care as required, including calling an ambulance.
